Have you considered flipping the plenum around so the the throttle body is on the other side?

If it's all original, the spark plug wires will have the date on them. Also, check the seat belt tag on the driver's seat for the manufacture date.

What year is the 1UZ?

I very much doubt it would make any difference, ITBs face upwards after all. The bolts holes won't align if you try and fit the throttle body upside down anyway.

Correct re: the radiator connections. There's a thermostat bypass under the housing that goes through the oil cooler (LS400) and connects to a port on the left side of the engine near the engine mount.

I'm pretty sure the 4/5 speed bellhousings are the same. Not so sure about the T/Cs but I would have thought they were the same. No idea about the Landcruiser 'box, you'll have to offer it up and see.

The only way you'll get the transfercase to fit the back of the Lexus five speed transmission is to get hold of or fabricate an adapter housing. Gloverman on here makes/sells them but they are very expensive. Alternatively fit a Lexus four speed transmission to the back of the VVTi motor.

Is the traction control TPS hooked up? If not then the flap could be restricting airflow in the throttle body.

Have you cleared the codes from the ECU's memory?

Ah OK, have you set the idle control switch to the stepper motor position? Any error codes logged?

Does the IACV have power? Do you have 12v at the +B terminal in the diagnostics port?
